Elective treatment fee for initial examinations

Patients who visit the hospital without a referral from another hospital or clinicare responsible for an elective treatment fee for the initial examination in addition to the regular medical fees during the initial examination. However, this excludes emergency hospitalizations, etc.

<Elective treatment fee for initial examinations>
This is a system established by the government (Ministry of Health, Labor and Welfare) to promote functional separation between hospitals and clinics. Regional medical support hospitals (except those with fewer than 200 beds) that perform an initial examination for a patient without a referral from another medical institution must assess the amount specified by the government on patients in addition to the initial examination fees. The elective medical fee is not required during the initial examination if the patient has a letter of referral.

Elective treatment fee for follow-up examinations

Patients who, by choice, continue to get examined by this hospital without a letter of referral and despite referrals to another hospital or clinic are responsible for the elective treatment fee for follow-up examinations in addition to the regular medical fees.
Elective treatment fee for follow-up examinations (medical): 2,750 yen (dental): 1,650 yen

For first-time patients of this hospital

When possible, obtain a letter of referral for this hospital from your family doctor.

Course up to examination

  • When you consider getting examined by a medical institution because of poor physical health, etc., first talk to your family doctor or a regional clinic.
  • If your family doctor or regional clinic recommends getting examined by a hospital, request the issuance of a letter of referral (Patient Referral Document).
  • If you have a letter of referral, the elective treatment fee for initial examinations (5,500 yen at our hospital) is not required when getting examined at the hospital.
  • To strive for health management and to create a cheerful hospital that grows with the region.
  • This is the course recommended by the MHLW for examinations. However, please note that we do not refuse first-time patients.

Examination hours

Examination hours 8:30 am to 11:30 am (reception from 8:20 am to 11:30 am) In some cases, no examinations will be held, or the examination will be performed by a substitute. When outside of reception hours (emergency outpatient examinations), please call for confirmation before coming.

  * The reception hours for orthopedics are 8:20 am to 10:30 am.  * If you have a letter of referral, contact us in advance.  * Be advised that, depending on the examining department, the examination hours, examination reception hours, and reservation/change of reservation hours, etc., may differ. For details, see the weekly examination chart. * If you prefer an examination outside of reception hours, contact us by phone for confirmation before coming.

Information for patients

(1) Handling of personal information

All personal information that we acquire through business operations is handled according to the “Act on the Protection of Personal Information” and “Handling of personal information by the Japanese Red Cross Society”. The purposes of use, etc., are posted in the hospital; if there are items that are difficult to agree with, let us know.

(2) Medical consultations

A full-time nurse will provide consultations and hearings on examinations for you or your family. There are no drawbacks for you or your family regarding consultations.

(3) Your family doctor

Find a family doctor to manage your daily health. Having a doctor that can provide advice and treatment on your health can provide significant relief. If your family doctor determines that you need specialized tests or hospitalization and they provide a referral for this hospital, it can make the examination go smoother as we will know about the course of treatment and prescriptions.

(4) Prescriptions for medication

Based on instructions from the government (Ministry of Health, Labor and Welfare) to promote the separation of medical treatment and pharmacies, generally a prescription for medication will be issued to patients who undergo outpatient examinations that is to be filled outside of the hospital. Get the medication from a pharmacy outside of the hospital that accepts health insurance. With the cooperation of the Azumino Pharmacist Association, we can fax the external prescription to a pharmacy that accepts health insurance. Feel free to use this service.

(5) Complete prohibition of smoking on hospital premises

Out of careful consideration of the negative effects that smoking and second-hand smoke pose to health, smoking is completely prohibited on the premises of the hospital. We appreciate the understanding and cooperation of you and your family.

(6) Disclosure of examination information

Patients can request the disclosure of their own examination information. We accept such requests at general reception. * If you have any other issues or questions at the hospital, feel free to ask the staff. ※その他、病院内で「お困りになったこと」、「不明なこと」がございましたら、ご遠慮なく職員に声をおかけください。

From consultation to home

1. Initial examinations (first-time patients)

General (initial examination) reception
For the initial examination, fill out the required sections of the Examination Application. If you have a health insurance card, recipient certificate, and/or letter of referral (including any images, etc.), attach them and submit the application at the general reception counter. If you have been issued a recipient certificate related to public contributions, submit that as well. When the reception procedures are finished, you will be issued an examination ticket and a reception slip at the general reception counter. Take the reception slip to reception of the specified block.

2. Follow-up examinations (second time onwards)

Use the follow-up examination reception machine or the general reception counter (follow-up examinations) When undergoing an examination for the second time, use the follow-up examination reception machine. However, the following patients should go to the general reception counter.
(1) Patients who do not have a reservation (2) Patients who have a letter of referral or film from another medical institution (3) Patients who have had a change to their insurance card (4) Patients who have lost or forgot their examination ticket
After completing reception with the follow-up examination reception machine or at the general reception counter, a reception slip will be issued. Take this to reception of the specified block. The reception slip shows your schedule for the day. Follow it as described. Also, the reception number is the 4-digit number on the top right of the reception slip. This number will be the same even if you have examinations with multiple departments.

3. When the examination is finished

Go to the general reception counter.
When the examination is finished, the basic schedule; an external prescription, if applicable; and your reservation slip for the next time will be put in your file and then given to you in the examination room. Show the file to block reception in the examining department, attach your examination card and insurance card, and give them to the general reception counter on the first floor. You will be issued a payment number slip.  Get the external prescription from the counter after checking the department name and your name. If you are being issued medical materials for home use, get the materials from B-block reception (counter for the provision of materials for at-home care), and then go to the general reception counter.

4. Patients with a payment number slip

Wait in the waiting hall until your number is displayed on the payment number display machine. If you were examined by multiple departments, a calculation will be made for each department, and the timing for when your number is displayed may vary. When your payment number is displayed, insert your examination card into the automatic settlement machine or put the barcode on the number slip against the machine, and pay the amount indicated on the screen.

5. External prescriptions

Take the external prescription that you receive from the general reception counter to a pharmacy outside of the hospital that accepts insurance to get your medication. At the external prescription fax guidance corner, we introduce pharmacies that accept health insurance, and we perform a fax-sending service for external prescriptions. If you have questions about the prescription or medication, let us know, and they will be handled by a pharmacist.

6. Patients receiving medication in the hospital

Use the intercom to get your medication from the medication dispensary window.

7. Patients with a subsequent reservation

You will be issued a reservation slip (A4 size) after the examination.

8. Examination tickets

The examination ticket can be used permanently for all examining departments. Keep it safe, and ensure you bring it to the hospital. There is a fee for re-issuing examination tickets.

9. Insurance cards

If you do not show your insurance card as a first-time patient, you will be fully responsible for the day’s examination fees. Ensure that you bring your insurance card. If you have follow-up examinations, your insurance card will be checked monthly on examination days. Show it to the general reception counter. If there are any changes to your insurance card, promptly inform the general reception counter.

10. Long-term care insurance, social insurance, medical consultations, etc.
(medical consultations)

We introduce welfare services, and we provide consultations on treatment fees, treatment concerns, cancer, medication, and nutrition. Consultations will be handled by the staff appropriate for the nature of the consultation. Consultations are free and confidential.

11. Documents and applications at the general reception counter

Talk to the reception counter if you have questions about the handling of public expenditures (medical assistance, rehabilitation for the disabled, care for disabled children, treatment for specific diseases, etc.), traffic accidents, work injuries, etc. Tell the general reception counter if you require a medical certificate. (Depending on the type, examination by a doctor may be required.)

12. Requests

Exercise sufficient caution by not placing personal effects and valuables in the chairs of the waiting rooms.

13. Reservations and changes to reservations for examinations and tests

Call reception for each block (varies by department) between 3:00 pm and 4:30 pm on weekdays.
